There are forms here for our work together. The first section of "Bring With You" are forms that you can fill out and bring with you to give us a jump start during our first session.
These are forms to print, fill out, and bring with you to our first session. ( Even better, f you can fill them out and send them back before our session.) If these seem straightforward sign them and bring them. If you have a question, or are uncomfortable signing, make a note on the form, don't sign it, and bring it with you so we can discuss your hesitation. NOTE: you will only need the "Release & Request for Information" form if there is someone you would like me to talk about your case with. For example: a past therapist or physician.
